Clinical quick reference
Drug: Paracetamol (acetaminophen) intravenous infusion. Reference concentration: 10 mg/mL (1000 mg/100 mL; some products 500 mg/50 mL). Route: IV infusion over 15 minutes; NOT IV push or IM. Confirm the actual Indian product label before use.
1. Indications
Short-term treatment of fever and mild-to-moderate pain when IV treatment is clinically appropriate; adjunct to opioids for moderate-to-severe pain in patients aged at least 2 years. The cited US label permits fever treatment in younger infants under age-specific directions; local Indian approvals may differ.
2. Adult and adolescent dose
| Patient | Dose and interval | Maximum in 24 h, all routes |
|---|---|---|
| Age 13 years or older, at least 50 kg | 1000 mg IV every 6 h OR 650 mg IV every 4 h | 4000 mg |
| Age 13 years or older, under 50 kg | 15 mg/kg every 6 h OR 12.5 mg/kg every 4 h | 75 mg/kg, no more than 3750 mg; single dose no more than 750 mg |
Minimum interval for these regimens is 4 hours. Count all oral, rectal and combination paracetamol products.
3. Paediatric dose
| Age | IV dose | Maximum total daily dose |
|---|---|---|
| 2–12 years | 15 mg/kg every 6 h OR 12.5 mg/kg every 4 h | 75 mg/kg, not over 3750 mg; single dose maximum 750 mg |
| 29 days to under 2 years (fever) | 15 mg/kg every 6 h | 60 mg/kg |
| Birth–28 days, gestation at least 32 weeks (fever) | 12.5 mg/kg every 6 h | 50 mg/kg |
Neonatal and infant doses are indication-specific. Premature infants under 32 weeks gestation and other unlisted groups require a neonatal specialist protocol; do not extrapolate.
4. How to calculate the exact IV volume
Formula: Dose (mg) = body weight (kg) × prescribed mg/kg. Volume (mL) = dose (mg) ÷ 10 mg/mL. For a 15-minute infusion, syringe-pump rate (mL/h) = measured volume × 4.
| Illustrative patient | Prescribed example | Withdraw from 10 mg/mL | 15-minute pump rate |
|---|---|---|---|
| Child 10 kg, age 2–12 | 15 mg/kg = 150 mg | 15 mL | 60 mL/h |
| Child 20 kg, age 2–12 | 15 mg/kg = 300 mg | 30 mL | 120 mL/h |
| Child 30 kg, age 2–12 | 15 mg/kg = 450 mg | 45 mL | 180 mL/h |
| Adult 45 kg, age 13+ | 15 mg/kg = 675 mg | 67.5 mL | 270 mL/h |
| Adult 60 kg, age 13+ | 1000 mg | 100 mL | 400 mL/h |
These are mathematical examples, not patient orders. Verify age, actual weight, indication, total daily exposure, fluid allowance and device accuracy. Pump rate shown is for 15-minute infusion of the undiluted reference formulation; use device-appropriate settings and local policies.
5. Step-by-step preparation for nursing staff
- Verify prescription, patient identity, allergy history, weight, age, indication, previous paracetamol doses and liver/renal risk.
- Check actual pack concentration (mg/mL), expiry, integrity and clarity. Do not use discoloured solution or one with particles.
- The 10 mg/mL product is ready to use: no powder reconstitution and no routine dilution are required.
- For a dose below 1000 mg, aseptically withdraw only the prescribed amount into a separate sterile syringe or IV container. Never hang the full 100 mL container for a smaller paediatric dose.
- Label syringe/container with medicine, total mg, total mL, concentration, patient and preparation time. Have an independent double-check for paediatric calculations per local policy.
- For small paediatric volumes up to 60 mL, the reference label recommends a syringe pump. Administer intravenously over 15 minutes, not by bolus.
- Use an appropriate IV administration set and ensure correct priming. Monitor the end of infusion to avoid air entry, particularly with a primary infusion.
- Do not add another medicine to the container or infusion device. Flush/manage shared access according to validated local compatibility policy.
- Discard unused solution from single-dose containers. For the cited vial product, administer within 6 hours of opening/transfer; check the actual brand’s instructions because container-specific requirements vary.
6. Dilution and compatibility
For the referenced 10 mg/mL formulation, dilution is not routinely needed. Do not arbitrarily mix with normal saline, dextrose or other drugs: any alternative dilution requires a product-specific compatibility and stability reference. Diazepam and chlorpromazine are reported physically incompatible in the cited label; do not co-administer simultaneously through an unverified common line.
7. Storage and stability
Follow the storage conditions on the exact purchased pack. Use aseptic handling and discard the unused single-dose remainder. The cited US glass-vial label specifies use within 6 hours after the seal is penetrated or solution transferred; do not generalise that window to every brand or bag.
8. Renal/hepatic, pregnancy and lactation
Contraindicated in severe hepatic impairment or severe active liver disease. Use caution with other hepatic disease, alcohol misuse, malnutrition and severe hypovolaemia. In severe renal impairment (CrCl at or below 30 mL/min), longer intervals may be warranted; individualise with the product label and clinician/pharmacist. In pregnancy and breastfeeding, assess indication and benefit–risk against current local product information.
9. Contraindications, interactions and monitoring
Contraindications: known paracetamol hypersensitivity and severe hepatic impairment/active severe liver disease. Review warfarin and other concomitant medicines. Monitor pain/temperature response, infusion site, hypersensitivity and liver tests when indicated. Adverse effects include nausea, headache, vomiting and rare severe hypersensitivity/skin reactions; overdose can cause fatal hepatic injury.
10. Medication-error prevention
Critical: distinguish mg from mL; do not confuse 10 mg/mL with other concentrations. Do not give the full 100 mL container to a child needing 15–45 mL. Count ALL paracetamol from oral, rectal and IV sources. Verify the total daily maximum before each repeat dose. Seek urgent toxicology advice for suspected overdose.
11. Indian practice caveat
Brand names and marketed strengths vary; the reference is US DailyMed prescribing information and does not prove Indian regulatory approval for every age or indication. Confirm CDSCO-approved pack insert, local formulary, neonatal policy and institutional administration guidance. No unverified Indian brand names are listed.
