Ampicillin–Sulbactam IV: 1.5 g and 3 g Vial Reconstitution and Infusion Guide

At a Glance

Formulation: Combination antibiotic containing ampicillin:sulbactam in a 2:1 ratio; a 1.5 g vial contains 1 g/0.5 g and a 3 g vial contains 2 g/1 g. Confirm beta-lactam allergy and indication.

Vial preparation

Vial Sterile water to add Approximate final volume
1.5 g 3.2 mL 4 mL
3 g 6.4 mL 8 mL

Reconstituted total concentration is approximately 375 mg/mL (250 mg/mL ampicillin and 125 mg/mL sulbactam). Note displacement: added water and final volume differ.

IV infusion

Further dilute immediately with a compatible fluid to a validated final total concentration of 3–45 mg/mL, then administer according to the product label, commonly over 15–30 minutes. The referenced adult labelled regimen is 1.5–3 g every 6 hours, with sulbactam not exceeding 4 g/day. Children require weight-based age-specific guidance; renal impairment requires dosing interval adjustment.

Nursing checklist

Verify vial strength and allergy, calculate both components, prepare aseptically, select manufacturer-approved fluid, label final concentration and infusion time, and inspect solution. Stability depends substantially on fluid and concentration: dextrose preparations can have much shorter stability than saline. Never assume an eight-hour stability rule applies to every mixture.

Warnings

Assess prior severe penicillin/cephalosporin reactions, infectious mononucleosis, C. difficile diarrhoea, hepatic effects, renal impairment, pregnancy and lactation. Use cultures and local stewardship protocols.

Expanded Clinical and Nursing Notes

Ampicillin–sulbactam contains two active ingredients in a 2:1 mass ratio. A vial marked 1.5 g represents 1 g ampicillin and 0.5 g sulbactam, while a 3 g vial represents 2 g and 1 g respectively. When checking an order, distinguish the total combination dose from the ampicillin component; a prescription expressed in one convention cannot automatically be interpreted in the other.

Before Preparation

Confirm indication, documented penicillin or other beta-lactam allergy, current kidney function, prescription frequency and culture results. Check the vial strength, expiry date, manufacturer-specific reconstitution instructions and permitted diluents. Use aseptic technique and label the prepared solution with drug, total dose, diluent, concentration, preparation time and required administration time.

Administration and Monitoring

Use the infusion duration and stability specified for the local product; do not infer them from another manufacturer’s label. Observe for immediate hypersensitivity, infusion-site reactions and diarrhoea. Reassess antibiotic choice against microbiology and local resistance data. Renal impairment may require an interval adjustment, and neonatal regimens are not interchangeable with adult schedules.
