Uttarakhand eSanjeevani Telemedicine Guide: Patient Access, Referral and PMHS Workflow

Current official service explainer | Original Uttarakhand NHM page dated 30 January 2025 | Updated and reviewed 29 September 2026

Uttarakhand’s eSanjeevani telemedicine service connects patients—especially those in remote hill districts—with clinicians and specialist hubs through government health facilities and online services.

Key points

  • Free government teleconsultation is available through the eSanjeevani ecosystem.
  • NHM Uttarakhand lists AIIMS Rishikesh, Doon Medical College, Government Medical College Haldwani, district services and other hubs.
  • Telemedicine is not a substitute for emergency transport or hands-on examination when clinically required.
  • Patients should use only the official MoHFW eSanjeevani portal.

How patients can access the service

  1. Open the official eSanjeevani portal or visit a participating Health and Wellness Centre/health facility.
  2. Register with accurate identity and contact details, following the portal’s current OTP process.
  3. Select the available service or obtain referral support from the local facility.
  4. Keep prescriptions, investigation reports and a current medicine list ready.
  5. Download and retain the consultation record or e-prescription when provided.

PMHS and referral workflow

The local clinician should document history, vital signs, red flags, current medicines and relevant investigations before escalation. A specialist teleconsultation can support diagnosis, treatment planning and referral, but the receiving facility’s advice and local protocol govern the next step.

Emergency red flags

Chest pain, severe breathlessness, stroke symptoms, major trauma, active bleeding, seizures, altered consciousness, obstetric emergencies and unstable vital signs require immediate emergency assessment. Do not delay 108 or local emergency referral while waiting for an online slot.

What is known

NHM Uttarakhand states that patients in remote areas can receive free specialist consultations through participating state and national institutions.

Limitations and uncertainties

Clinic rosters, specialties, appointment slots and technical access may change. Some conditions require physical examination, point-of-care tests or direct hospital referral.
