At a Glance
Drug: Clindamycin phosphate injection concentrate. IV concentrate must be diluted before infusion. Follow indication, susceptibility and the actual ampoule strength; not all premixed products need further dilution.
Labelled IV dilution examples
| Dose | Diluent volume | Minimum listed time |
|---|---|---|
| 300 mg | 50 mL | 10 min |
| 600 mg | 50 mL | 20 min |
| 900 mg | 50–100 mL | 30 min |
| 1200 mg | 100 mL | 40 min |
Limits: Final concentration must not exceed 18 mg/mL and infusion rate must not exceed 30 mg/min. More than 1200 mg in a single one-hour infusion is not recommended. The label provides adult total daily doses depending on severity and paediatric dosing generally 20–40 mg/kg/day divided in 3–4 doses for children 1 month to 16 years; neonates have separate regimens. Do not use these ranges without indication-specific prescribing.
Nursing example
For a prescribed 600 mg dose, verify the ampoule mg/mL, withdraw the correct volume, dilute into the authorised 50 mL volume, and infuse over at least 20 minutes. Recalculate pump rate from the actual final bag volume. Inspect solution, check line compatibility and monitor the patient.
Warnings
Important risk of C. difficile-associated diarrhoea/colitis, including severe or delayed disease. Monitor for hypersensitivity, hypotension with overly rapid administration and liver abnormalities. Pregnancy/lactation, neonates and organ dysfunction require product-specific review.
Expanded Infusion and Nursing Safety Notes
Clindamycin must be diluted before intravenous infusion; do not give the injectable product as a rapid undiluted IV bolus. The labelled maximum infusion rate is 30 mg/min, so the mathematical minimum infusion time is 10 minutes for 300 mg, 20 minutes for 600 mg, 30 minutes for 900 mg and 40 minutes for 1,200 mg. Check the specific product label for the minimum dilution volume and any per-infusion dose restriction.
Infusion Pump Example
If the actual final solution volume is 100 mL and the authorised infusion duration is 40 minutes, the corresponding pump rate is 150 mL/hour. The calculation does not determine whether a particular concentration is appropriate: verify both the concentration and duration against the product label.
Safety Monitoring
Monitor IV-site irritation, rash, haemodynamic symptoms during infusion and clinically significant diarrhoea. Clindamycin is associated with Clostridioides difficile-associated diarrhoea, which may occur during or after treatment. Review susceptibility data and local guidelines because clindamycin resistance varies by organism and location. Neonatal and paediatric regimens require separate age- and weight-based review.
